Medical Clearance form & see your Doctor or Medical Waiver
Federal law requires any person prior to being fit with hearing aids, must obtain a Medical Clearance from a licensed physician or sign a Medical Waiver declining the exam. (for more info., click here). We will Email, fax, or mail to you, or your doctor directly, a Medical Clearance form for your doctor to sign or a Medical Waiver form for you to sign.
At your own expense, your doctor may conduct an exam to insure there are no underlying medical conditions that need to be addressed. A Medical Clearance in the form of a simple "Script", similar to a regular prescription form stating that you are medically cleared to wear hearing aids is also acceptable.
The physician may have a hearing professional conduct a hearing test. If this is the case, please ask for a copy of your hearing test or Audiogram be provided to you. Be prepared for the hearing professional to try to sell you their own hearing aids. Please decline their invitation.
Under federally mandated HIPAA regulations, health care providers must provide you with copies of certain documents upon your request, including hearing aid clearance forms and hearing tests or Audiograms.
If you do not want a medical examination, Federal law allows for a
fully informed adult to sign a "Medical Exam Waiver Statement" declining the medical evaluation.

I have been advised by HearSource.com & any or all of their representatives, that the Food and Drug Administration has determined that my best health interest would be served if I had a medical evaluation by a licensed physician ( preferably a physician who specializes in diseases of the ear ) before purchasing a hearing aid.

The financial transaction will be completed using either Visa, MasterCard, Discover or Check by Phone for payment.We will Fax, Email or Mail back to you your purchase receipt and a Medical Questionnaire for you to complete and send back to us.

We'll build & ship your hearing aids
We will then build and pre-program your new HearSource hearing aids to your specifications. This process usually takes 3 to 5 business days. Your new hearing aids and accessories will then arrive via express mail at your home with easy to understand instructions.
